COVID-19

*Please note a change in location for COVID-19 testing for Active Duty from Preventive Medicine to the Military Medicine clinic.*
If you feel sick, have any of the following symptoms or have a positive self-test, please notify your chain of command, self-isolate, and contact Military Medicine at 843-794-6649/6652 to schedule an appointment during sick call hours for a laboratory confirmed test.  Appointments will be available Monday to Friday 0730 to 0930.
Symptoms: Fever, cough, difficulty breathing, sore throat, loss of taste or smell, congestion or runny nose, fatigue, muscle or body aches

Periodic Health Assessments

Individual Medical Readiness (IMR) elements include:

  • Periodic Health Assessments (PHA),
  • Deployment-limiting medical and dental conditions,
  • Dental assessments,
  • Immunization status,
  • Medical readiness and laboratory studies,
  • Individual medical equipment.
Annual Periodic Health Assessments (PHAs) will be performed at Navy Medicine Readiness and Training Command (NMRTC) Charleston Military Medicine Department, where all active-duty military are seen for care. Each service member must:

 Navy

  1. Complete the electronic Periodic Health Assessments (ePHA) questionnaire at: https://eha.health.mil/EHA/.
  2. Verify your Individual Medical Readiness (IMR) status online at BUPERS Online (https://www.bol.navy.mil/IMR) **ninth hyperlink down**. Ensure your IMR status is up to date before booking your PHA appointment. 
  3. Print off the IMR status page from BOL. *This is needed for booking*
  4. If there are any IMR discrepancies, visit or contact Dental (843-794-7944), Immunizations (843-794-6850), Occupational Health (843-794-6550), and Optometry (843-794-6881) to address discrepancies before booking an appointment.
  5. If the dental exam is not up to date, please schedule a dental exam. Service Member may contact the NMRTC Charleston Dental Clinic at (843) 794-7944 and select option #7.
  6. Fill out and print the Tuberculosis Exposure Risk Assessment Form and PHA Worksheet. Visit the Physicals Office for a final health record review and schedule an appointment with your Primary Care Manager (PCM).

 Marines

Army

  1. Complete and print out the online PHA assessment.
  2. Print out and bring your Medical Protection System (MEDPROS) report. Army: MEDPROS (https://medpros.mods.army.mil/MEDPROSNew)
  3. Fill out and print the Tuberculosis Exposure Risk Assessment Form and PHA Worksheet. Visit the Physicals Office for a final record review and schedule an appointment with your Primary Care Manager (PCM).

 Coast Guard

  1. Step 1: Confirm you are in your PHA approval window; the time period is 02 months before your birth month till the last day of your birth month. Ensure you have a current dental exam that is NOT due to expire within 60 days. Please report to the Dental Clinic if you are unsure or have questions about your dental status. Call NMRTC Charleston Dental at 843-794-7944. Your annual dental exam MUST be up to date before your PHA can be validated. Reservist, ensure you have submitted a current DD2813. Failure to do so will result in the termination of ePHA.
  2. Step 2: Ensure all vaccinations are up to date and not overdue. This can be verified by checking your personal tab in CGBI. Be sure to click "show details." https://cgbi.osc.uscg.mil/2.0/personal.cfm
  3. Step 3: Complete the ePHA Survey located at: https://data.nmcphc.med.navy.mil/PHA/index.aspx
 
***You must register for an account prior to survey completion*** Please have your DoD 10-digit ID number and your Unit Identification Code (UIC)/OPFAC on hand. Survey completion will take approximately twenty minutes.
 
a) Once the questionnaire is completed, personnel who receive ePHA services at NMRTC Charleston should EMAIL the required info to Base Charleston HSWL FLRS in the correct email format.
 
Do not schedule your PHA appointment until the Base Charleston HSWL FLRS has notified you. This gives time for the HSWL FLRS staff to review your medical record. Failure to do this can result in delay/ termination of the ePHA appointment.
 
Send an email to Base Charleston HSWL FLRS staff using both email links located above and below.
 
In "SUBJ" put "Active Duty Coast Guard PHA." Please include your DoD 10-digit ID number on the back of your ID card and a good contact number you can be reached at. We will conduct your health record review prior to your appointment. 
 
b) Personnel who are TriCare Prime Remote or a CG Reservist will receive ePHA services through the Reserve Health Readiness Program (i.e., Logistics Health, Incorporated, or LHI). Once dental is complete, send an email to Base Charleston HSWL FLRS staff using the links located above and below.
 
In "SUBJ" put "Remote Coast Guard PHA." In the body of the email, please include your DoD 10-digit ID number on the back of your ID card, overdue vaccinations, and a good contact number you can be reached at. Please be prepared to complete lab requirements as directed prior to your PHA appointment. A health record review must be conducted prior to booking your appointment.

**Reserve/ Remote members, please include overdue vaccinations in the body of the email so they can be ordered at the time of your PHA.

Operational Forces Medical Liaison (OFML)

Operational Forces Medical Liaison (OFML) / Fleet Medical Liaison serves as the primary point-of-contact for operational commands and units or individuals in the area of responsibility who do not have embedded military medical.  Tracks inpatients and intensive outpatients until the service member is returned to their unit.
