Periodic Health Assessments

Annual Periodic Health Assessments (PHAs) will be performed in Naval Health Clinic Charleston's Military Medicine Department, where all active duty military are seen for care.  The process for PHAs will remain the same.  Each service member must:

• First complete the ePHA questionnaire at: https://eha.health.mil/eha.
• Ensure their dental exam is up to date.  To schedule a dental exam, service member may contact the NHCC Dental Clinic at (843) 794-7944
• Verify their Individual Medical Readiness (IMR) status online at the respective service link:

  • Air Force: ASIMS (https://ft.asims.afms.mil/)
